Online Certification Application Home > Certification > Online Certification Application This application is for Professional Educator Certification with the Florida or International Association of Christian Colleges and Schools. (FACCS/IACCS). Only teachers or administrators who are employed at a FACCS/IACCS participant or accredited member school may apply for FACCS/IACCS certification. There are exceptions made for applicants who are applying to FACCS schools for employment to gain FACCS certification prior to being hired. When you complete and submit this application you will be instructed as to how to submit electronic copies of college transcripts (signed by the school Administrator), your Master Inservice Program transcript for ISP documentation, your proof of Philosophy of Christian Education completion, and fees. Processing will begin once the above mentioned documents and fees have been received but not before October 28, 2014 (as our office is offline for certifications during convention planning). ______________________________________________________________________________ SECTION ONE: PERSONAL INFORMATION Full Name (exactly as you want it to appear on your certificate) FACCS/IACCS School Name (at which you are employed, type NONE if not employed at a member school) Applicant E-mail Address (will be added to eUpdate newsletter list) Maiden/Previous Last Name (if applicable) If you hold an active or expired FACCS certificate and you have changed schools since your last application, list the name of your previous school. If you are NOT a faculty member at a FACCS member school, please provide your complete home address to which we will mail your certificate. ______________________________________________________________________________ SECTION TWO: APPLICATION TYPE Initial = have never held a FACCS certificate before Renewal = hold an active FACCS certificate and wish to renew it. Upgrade = hold a Temporary certificate and are ready to upgrade to a Professional certificate Additional = hold an active FACCS certificate and wish to add another certificate level or type Extensions = may only be applied for if your certificate has NOT expired Late = within 1 year after expiration Reinstatement = certificate has been expired for more than 1 year Type of Application (select one) Choose... Applicants for Renewal need only complete if information has changed since previous application was submitted or if ISPs were earned through college course work. Instructions to attach scanned original transcripts (required) will be provided after application is submitted. Each entry must include the following: 1) College/University Name, 2) City/State, 3) Years Attended, and 4) Degree(s) Earned.
